Workplace is not wheelchair accessible / barrierefrei.

Improve transparency, and communication. Avoid micromanagement, avoid enforcing strict hierarchies, and traditional superior/reporting systems. Move towards flat hierarchy, and increase employee involvement in feedback/review processes for higher employee trust & satisfaction.

Missing squads involvement in restructuring, hiring, firing, etc.

a little bit of micro-management but people are open to feedback, so can potentially be fixed sooner or later.
