Lack of clear direction or strategy from the top down. Poor decision making. Managers chosen because they won't challenge or rock the boat. Demotivated technical resources. Dishonesty to interview candidates. Infighting between departments. VERY political.
None, my direct manager had been hired due to being the same nationality as his boss and because he would not challenge incorrect decisions. I received no support from him,or HR when issues became obvious.
Infighting between departments, no collaboration.
None, micro management is rife due to incompetent managers trying to justify their salaries.
Employees are kept in the dark and challenges or straight talking are not welcomed.

Very scarce chances of promotion and internal moves are overtly blocked with no reasonable explanation.

My salary package had a fairly large percentage which was performance based. Due to personal disagreements between myself and my manager he was able to abuse his position to lower my bonus, even though my achievements were known throughout the department. When I approached HR to ask for a fair and unbiased review they were no help at all.

- Clear out of senior and middle management, hire externally to overcome the negative culture. Motivate demotivated employees and/or fire employees who are too demotivated to continue in a progressive way. Be honest during interviews and open with your employees, everyone can tell when a manager is lying. Do not include large bonuses in salary package and then allow someone with a personal bias to carry out the annual review.
Good office location, some great technical resources.
Poor senior management, culture of politics, infighting and dishonesty.
