Separation of management levels, people in management positions felt annoyed to deal with their surrogates.
Support from Management
Leadership was left to selected employees, who used that leadership in very irrelevant ways.
If you needed some help you would get it from another employee and not the leads, those took their time to come to you even if they were not busy.
Unrealistic goals given, work load too heavy, no help from leads, the attitude was "if you can't do it, well replace you"
