Constantly changing plans, make decisions without asking the people who do the work if it will work, what impact will be, etc.
The upper management divides and conquers. They have management staff with numerous complaints against them still allowed to be employed. Some have threatened staff, harassed staff, etc., which has been proven, but no action taken against the management staff involved.
