Supervisors and ops managers are too quick to focus on their fast track to success, and not the success of their agents.
Inconsistent in setting expectations. There were no firm rules, but referred to as "judgment calls". This is great in theory, but does not work if everyone is not all held accountable to the same standards of acceptable behavior.
Lots of shuffling agents around from department to department, lots of letting good people go for reasons that often seem random and surprising.
