You have to investigate why people are leaving! If you want authentic feedback, ask them one month or two after they have left (and got their work reference/certificate) about the reasons why they have left.

I had in my team correct and supportive colleagues and also ones, which I wouldn't go with for a lunch break! In this diverse team it was funny to observe how some try to practice "authority"

I had a manipulative team lead, who builded inner circles which from time to time causes colleagues to leave, because of the unhealthy circumstances. Not always the best people are appreciated but rather the average ones if those can make the "show"! In this regards it is not a problem for them that someone leaves as long as there is an HR which will recruit the next one!
