Primary Care Physician Coordination

We are seeking a persistent and detail-oriented PCP Coordination Specialist to join our Care Coordination team. In this role, you will be the primary point of contact between our organization and Primary Care Provider (PCP) offices. Your goal is to ensure the seamless retrieval of critical patient documentation through high-volume outbound calling, meticulous tracking, and professional follow-up.

The ideal candidate thrives in a fast-paced environment, is comfortable with repetitive high-volume outreach, and possesses the professional persistence required to meet strict deadlines.

• Provider Outreach: Conduct a high volume of outbound calls to PCP offices to verify patient status and confirm accurate contact information (phone, fax, address).
• Documentation Management: Confirm receipt of requests, resend missing documentation, and identify/coordinate corrections for incomplete forms or missing signatures.
• Pipeline Tracking: Determine the status of pending documents, establish expected completion timeframes, and follow up consistently until all signed documentation is returned.
• Detailed Record Keeping: Accurately document every interaction, status update, and follow-up action within the company system.
• Compliance & Collaboration: Maintain strict HIPAA compliance regarding patient confidentiality and communicate internal barriers or delays to team leadership.
• Performance Goals: Meet or exceed established productivity metrics, call volumes, and documentation return rates.

Dein Profil

• Education: High school diploma or equivalent required.
• Language: Fluent in English.
• Experience: Previous experience in healthcare, medical records, patient coordination, or call-center operations is strongly preferred.
• Technical Skills: Proficiency with CRMs, task-management platforms, digital fax systems, and Microsoft Office or Google Workspace.
• Communication: Professional, confident, and persistent telephone presence with the ability to remain courteous during difficult conversations.
• Organization: Exceptional time-management skills and the ability to manage a large volume of open requests simultaneously.
• Persistence: A 'get it done' attitude with a commitment to consistent follow-through.
• Accountability: Ability to work independently and take ownership of deadlines.
• Precision: High attention to detail in both verbal communication and data entry.
• Problem-Solving: Ability to navigate transfers, voicemails, and administrative hurdles to reach the correct contact.
